Instructions for completing the Disclosure Scotland Form

Please read the Notes of Guidance provided with the Disclosure Application and follow their instructions. The following instructions are specific to the University of St Andrews.

  • Part A1 - Enhanced disclosure should be selected.
  • Part B - All sections to be completed as outlined in the guidance notes.
  • B19 - B25 - Please bring three forms of identification with you as outlined on the form (one of these must include your current home address).
  • B30 - B58 - Please provide a full five years address history.
  • Part C 1/2 - Sign and date the form.
  • Part D - Leave blank.
  • Part E - Leave blank.

What to do with the form

You must bring this form to Human Resources on or before your first day of employment. Please ensure that you bring the original documentation outlined in E13 for verification. A copy of this will be taken for reference and the form will be retained by Human Resources for submission to Disclosure Scotland. What happens next? Once the check has been completed, a copy of your clearance certificate will be sent to you at your home address. A copy of this certificate will also be sent to the University and retained by Human Resources for no longer than 90 days. The certificate number and type of Disclosure will be entered onto the Human Resources database for future reference.
